Please complete a separate form for each student participant. After you submit the form on-line you will receive an e-mail with your participation agreement filled out. After you and your student sign and submit the online agreement please pay your commitment fee via PayPal or check in the  White Mailbox. Please do not pay in cash or give payments to the directors or leave payment on their desks.

Siblings may have a different payment schedule than returning members and a discounted fee. Note that one student per family pays the normal rate and other students in the same family receive the discount.
