PCMB LogoPCCS Activities and Athletics HandbookHere are the forms necessary for the 2017 marching band season. Please submit all these forms by Friday, June 23rd, the last day of the June band camp. The only exception is the physical form, which is due on Tuesday, September 5th.

These forms can be placed in the white mailbox in the large band room, or can be turned in at the mandatory parent meeting taking place on Wednesday, June 21st at 7:00 p.m. in the Plymouth High School cafeteria. Please do not hand your forms to the directors or place them on their desks.

Please print the two page forms two-sided on a single sheet of paper. Also, please do not staple the pages together. Please either paperclip them, or better yet, place them in an envelope to make sure the pages do not get separated.

PCCS Activities and Athletics Handbook

Students of the P-CEP Marching Band are expected to adhere to the rules spelled out in the athletic handbook. This document needs to be read, understood and  followed. It does not need to be signed or turned in.



Permission Slips

There are 6 permission slips plus a nurses medical form required for the 2017 season. The document below can be filled out on-line and printed. Filling out the first cover page will automatically fill out all the permission slips as well as the extra medical form for the nurses at the end. Be sure to print these single sided only because they each need to be filed separately. Forms printed double-sided will be returned to you. The cover page does not need to be turned in. Remember you must sign ALL of the forms before turning them in.


Field Trip Information Sheets

The Field Trip Information Sheets contain additional details associated with the field trips for your reference. There is no need to sign or turn in these documents.


Ford Field Forms

Ford Field and the MCBA requires all students who perform at the State Championships and all parents who volunteer for the event to complete this form. Note the following when completing the Ford Field forms:

Parents and guardians - we ask you to submit the Parent Form for each adult even if you do not plan at this time to volunteer at this time. This will make it easier to manage the paperwork, especially if your plans change.



The physical must take place after April 15, 2017. If you have a physical form from another sport dated AFTER April 15, you can turn in a copy of that form. The form must be submittedon Tuesday, September 5th.


Volunteer Interest

Parents are encouraged to participate in their student's success by supporting the organization that supports these young musicians. The Volunteer Interest form can be found under Booster Forms.



Volunteers who come in direct contact with students during any school sponsored event is required to submit an ICHAT form for a district-mandated background check. The form and additional details are found under Booster Forms.
